2020: US military finally runs out of fuel, making it logisiticaly impossible to maintain completely control of the continent. The US Navy positions nuclear vessels around the country to impose order.

2021: The President of the United States signs the Morgan Acts, dividing the administration of the country into four Military Governed zones: West, East, Mid-West, Mountain. Formal end of the United States of America as a single entity

2030: President Bryan nationalizes the Coal Mining Industry. Coal burning rail lines become common on the Eastern Seaboard.

2042: United Cities of Washington secede from the Western United States, becoming the first successful independence movement in the Continental United States. Alaska and Hawaii leave the union shortly thereafter.

2051: Texas secedes from the Central United States after the Sheirffdoms of Austin, Dallas, Fort Worth, and Houston sign the New Republican Compact

2099: Formal end to the Western United States after California’s independence.

2167: Establishment of the Baptist Kingdom of Alabama, Eastern United States. With only the Eastern United States remaining President Bryan III is granted new emergency powers by the Congress, establishing the Presidency as a hereditary line only requiring elections in the event that an heir is not produced. The Washingtonian Empire is born.

2194: Establishment of the Southron Confederacy. Start of the First Carolina War

2245: First Carolina War ends, Southron Confederacy retreats East of the Appalachians.

2316: Beginning of the Second Carolina War.

2339: Southron Confederacy collapses, the Empire's conquests secure the Gulf.

2344: Birth of the Second Christ

2419: President Adams V captures Chicago. The Empire now controls most of the territory East of the Mississippi. This is the greatest extent of the Empire's borders.

2429: Southern Carolina successfully leaves the Empire, cutting the Washingtonians off from the riches of Florida and the Gulf.

2501: Massachusetts breaks off from the Empire, under the Kelly Dynasty.

2611: President Reagan IV declares Evangelism the State Religion of the Empire.

2655: The Jersey War ends with the Treaty of Hoboken. The claims by the Empire of N'York are ceded by the Washingtonians.

2712: End of the Third Mormon Campaign, Angeleno Knights return from Colorado with ancient holographic storage devices with knowledge left from before the Fall of the Ameircan Empire. Beginning of the Second Renaissance.

Its difficult to say, but generally speaking the Empire, N'York, and Kells are are always vying for dominance of the region, but none can realistically do more than capture the same border territories from eachother.

I gather from what I'v read and putting two and two together that monarchy is the most common form of government once again or at the very least only really competes with theocracies ?

Yes, as is typical with societies on the verge of collapse, a few powerful people come to be the ones in charge.

How common are other forms of government such as Cascadias meritocracy or is that a unique case ?

Its not a meritocracy its a Technocracy, but even it suffered from autocratic rule. Only recently did it get the reforms to make it a Technocratic Republic. There are a few city-states with similar forms of government, but for the most part, Autocracy is the the go-to system of government by these nations. Though to be fair, very few of these nations are Hereditary monarchies, almost all of them are elective monarchies.
